When using QuickView, the document pages start appearing as squares, resembling a puzzle missing pieces. Why does this happen?

This is a known issue.

This happens when your browser loses connection to the document while you're viewing it. Close out the QuickView browser window and re-open it, and it should appear normally again. Our Engineers are working to correct this oddity.
